Media Usage Permission and Current Physical

As we are approaching the end cheerleading camp, I have material that I would like to post of the squad, however, I am unable to because I don't have everyone's media usage form. I need those filled out and returned. If you don't want your image used on the blog, fill it out and deny permission. Otherwise, fill it out and grant permission. Remember, I have to have a current physical on file prior to practice starting again in Septemeber. Please turn those physicals into me and I will make sure that they get to Jeaneen in the office.
